Provider Application
NEW — Effective February 1, 2009, the Missouri Department of Insurance requires all healthcare providers to utilize the Council for Affordable Quality Healthcare for credentialing. It is necessary for providers who apply to print the CAQH form for completion and mail it to our office. This application will be the only form accepted. Commercial payers are not compelled to obtain access to the CAQH Universal Provider Datasource.
