Community Health Plan offers a Preventive Dental Rider administered by Essex Dental Benefits to the employees of groups who purchase it as a part of their medical benefit and as a benefit under our Advanced Choice Value Plus individual product line.

If your benefit plan includes the Preventive Dental Rider, you will receive a separate ID card from Essex that includes customer service contact information.

This rider covers preventive dental services, such as oral exams, cleanings, and X–rays up to twice per year. It does not cover basic and major services such as fillings, periodontal services, crowns and dentures. For a complete list of services and exclusions please click here.

Benefits are payable after a $25 office visit co–payment. If you receive treatment from a participating dentist, the office visit co–payment is all you pay for covered services. If you receive care from a dentist who does not participate in the Essex program, you will be reimbursed up to the scheduled limit, less the $25 office visit co–payment for covered services.
