Community Health Plan has partnered with the Beech Street Corporation to provide you with a better out–of–network benefit. When choosing to access a non–participating provider outside of the Community Health Plan service area you are encouraged to access one of the thousands of Beech Street providers throughout the country. A list of these providers can be found at www.beechstreet.com or you may contact Beech Street via telephone at (800) 877-1444. The Beech Street logo appears on the Community Health Plan Member Identification Card and is easily recognized by providers. Please note: This benefit applies to PPO coverage only.

All services received from Beech Street providers are subject to your non–participating provider benefit. However, your deductible and co–insurance will be based upon the applicable Beech Street contracted rate, which will typically be less than the rates charged by a non–participating provider that has not contracted with Beech Street.
