About CHP
In 1994, a group of northwest Missouri citizens gathered to discuss what was needed to maintain the quality of life and the strength of the region during the coming years. This group included legislators, health providers, business representatives and community residents. Although they looked at many areas, health care was an area that stood out as impacting the health status and economic vitality of the region. Their recommendation was to develop a locally owned health plan where employers, health care providers, and the insurance provider were in partnership to provide high–quality, cost–effective health benefits to northwest Missouri and northeast Kansas residents.
From these ideas and efforts, Community Health Plan was launched in July 1, 1995.
We offer health insurance benefits to employer groups with two or more employees. We offer health care coverage that is locally based. That means:
- local claims handling
- local customer service
- local doctors

local decision makers
Our Board of Directors is made up of people who live and work in this area. Our physicians are the same ones you have already come to trust. The hospitals, pharmacies and clinics in our network are already familiar partners in our medical community.
Community Health Plan is a non–profit, locally based plan. That means we answer to the needs of our members, not to stockholders.

To provide our customers high quality and cost competitive health services through locally delivered managed systems and financing mechanisms which maximize the efficient and effective use of available resources and improve the overall health of the population.
HEDIS Ratings
As part of a Health Improvement Organization, Community Health Plan continually monitors quality. We publish our most recent ratings from Health Plan Employer Data and Information Set (HEDIS) — a broad–based set of measures, designed to assess health plan performance in all areas that are important to purchasers and consumers of managed health care.
