Member Rights and Responsibilities
As a Community Health Plan Member, you have:
- The right to receive information about Community Health Plan, its services, its practitioners and providers, and members’ rights and responsibilities.
- The right to be treated with respect and recognition of your dignity and your right to privacy.
- The right to participate with practitioners in decisions about your healthcare.
- The right to a candid discussion of appropriate or medically necessary treatment options for your conditions, regardless of the cost or benefit coverage.
- The right to voice complaints or appeals about Community Health Plan or the care it provides.
- The right to make recommendations regarding Community Health Plan’s members’ rights and responsibilities policies.
- The responsibility to supply (to the extent possible) information that Community Health Plan and its practitioners and providers need in order to provide care.
- The responsibility to follow the plans and instructions for care that you have agreed upon with your practitioners.
- The responsibility to understand your health problems and participate in developing mutually agreed–upon treatment goals to the degree possible.
