Community Health Plan is committed to providing the highest quality services to our members. Community Health Plan ensures that this commitment is met by developing and implementing our Quality Improvement Program. The program is dedicated to continuously address customer’s needs and improve the quality of care and service provided to Community Health Plan members.

This program is based on the Plan’s strategic initiatives and mission. The program focuses on high–leverage/problem–prone areas and utilizes a systematic method of improvement and data collection to make decisions and monitor performance, based on best practices/benchmarks and is supported by Community Health Plan leadership.
