Care Partners — for employer groups
 When companies choose Community Health Plan for their health insurance needs, they are each assigned a care partner, a registered nurse who works with members who have been diagnosed with high–risk conditions such as diabetes, heart disease, asthma, chronic obstructive pulmonary disease (COPD) and depression (for more information about these programs, visit Community Connections Disease Management). Care partners are specially trained to coordinate care and services and provide education and support. Care partners work with those with high–risk conditions to ensure that they have all the tools and resources they need to improve their health and quality of life.
